Im a T and H sufferer so not without knowledge but I have a question for a friend.
They have T and H, they are nearly housebound. They are scared because every time they venture out, with ear plugs at the ready, any siren, car alarm, bus horn gives them yet another new T sound. No professional has a clue why this happens every time. Now they are understandably scared to go out as even quickly inserting plugs which are loose in their ears does not stop the new sounds. To this person the T sound is very important, as I know some folks with H say they don't care about T at all.

The problem is why does it happen, she is staying in more and more afraid of new noises. She really can't cope anymore

Try an in-the-ear sound therapy device long term (they are adjustable in level of sound provided); they can also amplify if they have hearing loss. The soft, constant noise provides backdrop so that sounds don't have so much contrast. The noise they generate can be very soft or loud.

I'm afraid my friend tried them and said it made H worse. She really is locked in a bad spiral.
But I will suggest what you say as I understand what you mean about contrast of sound

From the little you have said, it sounds like your friend is locked in a cycle of escalation, where unwanted perception causes anxiety causes increased sensory acuity, which in turn causes a greater perception of the tinnitus/H, which causes more anxiety and ever more increased acuity...

My guess is that she needs good, professional, therapeutic support of some kind (and I don't mean someone who will just throw drugs at her, that's more likely to do harm than good IMO).

Whenever anxiety conditions of any kind lead to a degree of agoraphobia that prevents normal day-to-day functioning, things seem to get really bad in a hurry. She may not be able to "think herself out" of her situation without a lot of hand-holding and support.

If she could find a psychologist who has experience treating people with severe sensory issues, that might be very beneficial. The usual sort of lifestyle modifications that I suggest for tinnitus/chronic pain, are not likely to be helpful to someone who has already escalated into a severe situation.

      Hi Noise box ,
Is the person off the BTA first initial B ?

She really needs lots of support now from Specialists as the loud sounds create new sounds due to the stress .
I know this person had trouble with the 4th of July fireworks and Bonfire night not far away .

Maskers might help her and a understanding friend to go out for walks to start off getting out and about.

I would not recommend mp3 players but if they help her set really low they might help out side...
Nice to know she has a friend like you.
Try get her join here as its world wide and might be someone near to her ....lots of love glynis x

This article was in a dutch newspaper, this was the best translation i could do!
Through internet Loon joined the rehabilitation for hearing disorders in Ghent , where he was subjected to a special therapy by audiologist Katleen de Waele . The most drastic treatment were two ' noise generators ' , hearing aids, which neutralized the ringing in his head . Van Loon carried the noise generators and 2.5 years until early 2013 he could completely take off them . "The noise generators neutralized the pain. Hyperacusis has nothing to do with hearing loss but with the brains . Through years of treatment, the brains are again accustomed to sounds. I can hear again like before , I have my life back . I'd get anti-anxiety meds at this point.

Yes, hydroxyzine is not ototoxic and tell her to visit an audiologist for the in the ear latest. You can get it down to a scarce whisper you can barely hear.that is user controllable.

No Glynis it's not, this person is not on forums or groups. Very private person. But so trapped in a vicious cycle of fear. I'm trying hard for her, we talk each day, but with my H I'm a bit stuck too for going to them for walks as I would have to drive 30 minutes to them.

Its hard I know and how tough its been and still is for you .
The person needs the right team in place and support from the medical profession and her own action plan to move her in little step forward and maybe medication involved .
She must be in a very low and vulnerable state and lonely.
Her Hyperacusis needs treating first with TRT and Maskers and CBT and the people involved in her action plan...lots of love to you both...glynis

Oh Glynis, thats the problem she can get maskers but did not get on with them. But being in the same area as me cannot get TRT or even CBT. She has just drawn a blank trying to get either. Its so frustrating.

Aww no....it's so hard when the services are not in your area to support tinnitus sufferers and makes it harder go further afield for help when suffering so badly and the winter also approaching to add to it.....bless her .

Do you have MIND in your area to have 10 free 50 minute sessions of counselling or phone counselling could be a option if she is ok to cope with a phone .......lots of love glynis
