Do you mean TTTS as a sharp, immediate pain that happens with sudden sounds, even if they are not necessarily loud, like the sensation of someone sticking a needle in your ear?

Each case is different. Personally, I avoid using protection when I am home alone, since I have control over the sounds in that environment.

Tinnitus does what it wants, when it wants. Protect your ears; they are as fragile as glass. Tomorrow will be a better day.
