Ear Pain without Hyperacusis?

From what I've read on the forums, it's common for people to experience ear pain and fullness with the onset of tinnitus. I've had tinnitus for 6 months and I still have ear pain. It's just a sharp jab, usually happens 5-10 times a day in my right ear and 2-3 in my left. Doesn't correspond to noise exposure. ENT said my ears look fine, no inflammation or signs of fluid.

Any ideas?

 

Tensor Tympani Syndrome?

 

From what I found online, it sounds like the TTS response is triggered by sound. Not the case for me. Doesn't feel like a muscle spasm either. Good try though, I'd never heard of it before.
