      I started having very loud pulsatile tinnitus in my right ear about a week ago.

      I had a mastoidectomy in this ear in 2012 and my surgeon has since retired.

      I tried to make an appointment with several ENT Specialists in my area but none will see me without a physician referral. How aggravating!

      Now I will have to pay an additional office call for a referral.

      My retired ENT specialist saw me without a referral.

      I even offered at one office to pay cash for the visit instead of using my health insurance at one office but they still would not see me.

      So I guess I will have to go to 2 doctors and pay 2 office calls for my problem. How aggravating!!

      Wow, that is aggravating. On the positive side, this might be a problem your internist can help solve. I don't know much about pulsatile tinnitus, but maybe the cause has something to do with blood pressure or anemia or thyroid or just something else going on that's not specifically ear related. Also, if the first doctor can't help you maybe he/she will be able to direct you to a very good ENT or otologist who actually cares about tinnitus.
