In the early months after onset I tried often to suppress all noise with plugs and isolation. But if I don't have at least the slightest ambient noise besides my T I start to go mad. I can hide from the world but not that awful ringing.

I thought about it a few times on adding a family member on all my accounts but T has made me too bitter and vindictive. If it gets to that pt, I'm just going to cash everything out and burn all the money in my back yard, even if it's illegal to burn money. I'm at a pt where I don't give F. No one is getting anything if I decides it is the end. Taking it all with me. lol.

Your brain's auditory cortex is active during anesthesia and if the surgeon likes to play loud music or a tray of surgical instruments hits the ground, then a spike can occur.

Your ears do not shut off when asleep or unconscious, it's the body's safety mechanism - meaning, in theory, you still 'hear' what you normally would - it's just whether your brain and body response to tinnitus acts in the same way.
That said if it rains through the night and heavy, I will often have a spike in morning, even if I didn't or rarely woke up to it in the night

@Jenny_S almost nobody wears earplugs to concerts. If you go to one, look around. IMO Loud noise does not cause permanent tinnitus. It is a factor in causing temporary T in my opinion - but it is something else that causes it to stick around permanently. We do not know what this is. If loud events caused permanent T, almost everyone would have T.
