Buzzing in One Ear, Worse When Lying Down. Boston Area Doctor Recommendations?

I have a buzzing noise in one ear that was damaged 35 years ago. The buzzing started about 4 years ago. It is often triggered by lying down, and becomes worse during the night. I wear an ear plug to tolerate it. But sometimes it can be constant for hours during the day also.

Any recommendations for a good doctor in the Boston area? I need to figure out if this is tinnitus or an inner ear spasm. The latter can possibly be treated by cutting a muscle, but I need a doctor knowledgeable enough who is willing to try it.
