College Survey for My Thesis on Tinnitus

I'm an Irish student doing my thesis on the internet and how we sometimes use it for personal health research. I've focused my research specifically on tinnitus (as I have tinnitus).

I would really appreciate if you could do this very quick survey for my research into tinnitus and the digital age.
