I drive a 644 John Deere bucket loader for garbage co.

What type of ear defender would be enough for a really loud environment? Loud back up alarms.

I have medium hyperacusis and loud pulsatile tinnitus 6/10.

I have not been to work since onset 3-17.

It is my understanding that 3M Peltor X5A provides the most protection (29 dB). If you wear ear plugs under the muffs, the noise reduction rating might go up by another 5 dB to 34-35 dB. But that might not be enough! If you get a spike after you work for one day, I hope you will be able to find another job...

Bill Bauer gave you good advice. I totally subscribe what he said. Double protection is your best option, see how it goes.
