Cause of Tinnitus
Several causes
There are a few movies with sequences about tinnitus:

- A Star Is Born
- Love & Mercy, about Brian Wilson, from the Beach Boys

There's also a movie from 2007, with Tim Robbins, called "Noise", which is apparently about a New Yorker who is against the noise of sirens, etc... I haven't seen this one...

Have you seen any movie about tinnitus or hyperacusis?

I havn't seen it but apparently the main character in Baby Driver has tinnitus.
It's a good movie, but it's overall a poor representation of severe tinnitus. It's mostly an excuse just to explain why the protagonist is constantly listening to music. (Masking.)

As for hyperacusis, there's an episode of the Rod Serling Twilight Zone that seems to be about it in everything but name. It's about a millionaire who constantly keeps everything loud all day, and then even the slightest noise starts sounding too loud for him.

Has anybody seen Cop Land, starring Sly? Sly plays a sheriff who suffers from tinnitus, but it doesn't pose an obstacle for him in dealing with corrupt policemen ;)

The main character in Dallas Buyers Club has episodes of severe fleeting tinnitus, either from stress, drug use, AIDS, or a combination of all three.

At first I thought it was just an artistic audio choice meant to reflect his mental state, but he acknowledges it in one scene: "I got a goddamn jack hammer going off in my head."
