Nose Spray for Eustachian Tube — Feels Like Something Inside Ear Is Moving — What to Do?

I went to a doctor about a month ago complaining about full ears, but had ear wax removed not so long ago, he prescribed me nose spray to drain my middle ear, after a month of using it I feel like my tinnitus has decreased in volume a bit, but whenever I go to sleep I still feel that there is something inside moving, and whenever it moves my tinnitus either goes very low in volume or switches ears...

What should I do? Go back to the doctor again? Is there anything he can prescribe?
