I've seen that erythromycin is ototoxic thru IV and pill. I was prescribed erythromycin as an ointment to put in my eye. Would it still be considered ototoxic? My ENT is away and my ophthalmologist isn't sure.
The problem with Erythro seems be dose related i.e rate of rise in blood levels. IV dosing is worst. I'd be surprised if an ointment was able to bring your blood levels up to anything like toxic. That said, ask for an alternative if you can. There's no reason to tweak the tail of the tiger if you don't need to.

I just had a cyst removed from my eyelid, and they applied Erythromycin ointment to the area. Three hours later, my tinnitus has spiked severely. I wiped the ointment away.
Is there any chance that my spike will go back down, or could this be permanent? Oh my God.
